The unit sees many types of diagnoses with the most common being related to: CHF, Stroke, Irregular heart rhythms, COPD, Chest Pain, UTI, Sepsis and Kidney Failure. The 4 TELE staff has been nominated and selected several times for The Daisy Award, an award you can only receive from a patient or family nominating you for going above and beyond expectations. The team of RNs is made up of a variety of experience from New Grad to 20+ yrs. of experience. We are a blue-ribbon unit that pride ourselves in giving the best clinical experience to nursing students. We also have integrated LPNs back into the nursing matrix. Our unit also has 22 specialty Hercules beds that saves our backs! Come join us, we would love to have you on the TELE TEAM!SUMMARY OF JOB RESPONSIBILITIES:The Team Leader serves as a clinical leader in the Nursing Department to ensure that each patient receives quality, individualized care that is proactive and coordinated among the care delivery team and that the patient is attaining their clinical milestones and patient flow.The Team Leader is responsible for assisting with the oversight of daily operations of the Nursing Unit. The Team Leader is responsible for the development and coordination of activities that promote clinically effective and efficient patient care. Plans, organizes and directs patient care activities under the direction of the nurse manager. Collaborates with the nurse manager in maintaining unit operations, supervises assigned personnel and consults and collaborates with other health care team members to provide safe appropriate patient care.QUALIFICATIONS:Certification in area of specialty within 2 yearsMaintain BLS, ACLS, and PALs as required based on unit competenciesAdditional requirements as required by unitEDUCATION:Current RN license in the State of Ohio.BSN preferred, or must be obtained within 5 years.EXPERIENCE:Minimum of 2 years Professional Nursing experience.KNOWLEDGE AND SKILLS:o Ability to analyze clinical, financial and social situations, identifying problems and alternative courses of action.o Willingness to be flexible, resourceful, and creative in problem solving.o Ability to act independently and offer suggestions and new ideas for improving performance and operations.o Ability to guide and direct the health care team to assure optimal outcomes. Clinical expertise in specialty area.o Ability to organize and present information clearly and concisely.o Excellent interpersonal skills in order to effectively communicate with physicians, patients, families and other members of the interdisciplinary team.o Creative problem solving and critical thinking.o Excellent customer service skills.PRINCIPAL JOB ACCOUNTABILITIESResponsible for timely assignment and facilitation of admissions based on established milestones. Anticipates and intervenes to assure patient milestones are met, including rounding with physician. Participates/facilitates in Quality Flow Rounds (QFR). Maintains working knowledge of proper patient type and consults with case management as needed for status updates. Collaborates with Case Managers, Social Workers and Staff Nurses regarding discharge needs/ barriers to discharge Supports implementation of new processes to improve patient care Identifies performance improvement opportunities and offers suggestions for improvement. Role models clinical excellence and demonstrates current technical and therapeutic skills. Assists in maintaining appropriate staffing assignments using budgeted guidelines. Assists with orientation, competencies, and continuing education for staff In collaboration with the Nurse Manager/Assistant Nurse Manager, enforces code of conduct and policies using effective and constructive communication skills. Assists with recruitment and selection of personnel as requested. Active participant in service recovery through investigation and intervention. Performs chart audits for accuracy and completeness. Works with other departments to coordinate patient care. Facilitates unit cleanliness and order and participates in environmental rounds. Evaluates the care provided by other members of the team.
