patient care? UPMC Washington is seeking a motivated and
knowledgeable Pharmacist to join our dynamic healthcare team. In
this vital role, you will contribute to the safe and effective use
of medications, collaborate with multidisciplinary teams, and
support our mission to deliver exceptional care to every patient,
every time. /n Responsibilities: /n /n /n - Interpret and evaluate
physician orders and prescriptions to prepare, calculate, and
dispense appropriate medication dosage forms for inpatient and
outpatient treatment in accordance with the Pennsylvania Pharmacy
Practice Act, Federal Drug Laws, and other regulatory
organizations. /n - Identify and report suspected adverse drug
reactions accurately and in a timely manner. /n - Sustain the
formulary by minimizing non-formulary procurements, utilizing
therapeutic interchange protocols, and promoting rational drug
therapy selection. /n - Review, maintain, and screen patient
medication records and profiles for allergies, drug interactions
and therapeutic duplications while entering orders and/or verifying
medications. /n - Organize daily operational duties and assist in
the supervision of pharmacy technicians, pharmacy aides, student
interns and other ancillary personnel involved in providing
pharmaceutical care. /n - Adhere to all regulatory requirements,
professional standards of practice, and hospital quality management
policies and procedures in providing pharmaceutical care. /n -
Communicate with physicians and nurses to ensure prompt
clarification of orders that are unclear, illegible, or incomplete.
/n - Provide drug information and/or educational programs to
healthcare professionals and pharmacy students. /n - Prepare and
dispense intravenous admixture solutions, as required, using
aseptic technique in an accepted sterile product preparation area.
/n /n Preferred Qualifications: 1 year hospital pharmacy experience
OR board certification. /n Schedule: Rotating weekdays, weekends,
and holidays. Primary shifts will include: 7AM-3PM, 3PM-11PM,
11PM-7AM, but could be pulled to other shifts as well. /n -
Graduate of an ACPE accredited College of Pharmacy. /n - B.S in
Pharmacy or Doctor of Pharmacy (PharmD) degree. /n - Required:
Current licensure as a Pharmacist in practicing state. /n - Entry
Level: no previous experience required; pharmacist experience
preferred. /n /n Licensure, Certifications, and Clearances: /n /n -
Pharmacist /n - Act 33 with renewal /n - Act 34 with renewal /n -
Act 73 FBI Clearance with renewal /n /n UPMC is an Equal
