Nurse to patient ratios are 1:5 during the day shift and 1:6 during the night shift. The maximum ratios can go to 6:1 during the day and 7:1 at night. Common diagnoses include post-operative orthopedic cases such as knee and hip replacements, shoulder replacements, and fractures. Other types of patients include those recovering from general surgeries like hemicolectomy, cholecystectomy, appendectomy, and hernia repair. Patients post-cardiac interventions and those requiring dialysis catheter insertions are also treated on this unit. Charge nurses are present on each shift. Daylight charge nurses do not take patient assignments, while night charge nurses may take a small assignment of 2-3 patients. Patient care technicians are available on the unit. During the day shift, there are 3 technicians, and there are 2 technicians on the night shift. The duties of the technicians include bathing patients, changing linens, assisting with feeding, ambulating patients, taking vital signs every four hours, and documenting in the electronic medical record. They also manage intake and output for various tubes and devices. Respiratory therapists administer breathing treatments and assist in managing respiratory equipment such as CPAP and BiPAP. Nurses troubleshoot equipment but do not operate ventilators. There are no monitor techs on the unit; nursing staff do not read their own strips. Interpreted telemetry strips are sent to the unit for review. Shifts are set with no self-scheduling, and adjustments are made as needed to accommodate vacations and holidays. There is a requirement to work every other weekend. Staff may be asked to participate in holiday rotation based on a predetermined schedule. There is no on-call requirement; standby is only for low census situations. Staff may float to other medical surgical units if required. Nurses are not provided with scrubs; navy blue scrubs are required for nursing staff. A specific orientation period is offered with an educator and a preceptor. Cancellation policy allows the client to cancel a limited number of shifts during assignments. The charting system is Cerner. A BSN is not required for this position.
