comprehensive treatment. As a Case Manager , you play a direct role
in saving the lives of our patients each day by providing and
promoting an environment of long-term recovery. You will partner
with the Clinical Director and Executive Director to establish and
maintain a therapeutic environment and routine for our residents.
Requirements: Bachelor’s degree preferred in the Human Service
field from an accredited college or university. Previous experience
in case management and/or utilization review. (preferred)
Responsibilities: Assess new referrals for appropriate level of
care referral. Determines need for therapeutic medical,
psycho-social, and psychiatric evaluations. Coordinates services
with counselors and medical case managers. Monitors cases by
verifying patient attendance, observing and evaluating response to
treatment, advocating for needed services, obtaining additional
resources, intervening in crises, and providing widespread support.
Assist with discharge plans and aftercare referrals. Benefits: 18
