Social Service DesigneeCommunity Skilled Care Centre is a non-profit nursing home focused on our residents and our caregivers. Our property is also home to The Commons Assisted Living. Responsibilities!

Intakes, develops, creates individualized care plans, MDS, maintains recordsWorks with Residents/Residents' families/responsible party, community, facility leadership teamWorks to ensure both advocacy and clinically driven outcomes, council concernsWorks to ensure understanding via education, referrals, supports safe dischargeTeam position, cross coordination in driving care outcomes with Clinical Care Coordinators, Risk and QAPI to ensure programming execution, intervention, and supporting documentation remains ongoingRelays information via care conference, IDTQualifications!
